Thank you for using our Web site to find out how to change the bank account that we use to withdraw your weekly invoice settlements.  Below is a form that must be printed, completed and returned to the Michigan Lottery to change your EFT bank account.  Please complete the form, then fax (or mail) the form to us, along with a void check or deposit slip that has your new routing and EFT bank account number preprinted on it.  Please make sure you sign the EFT form.  Your signature gives us authorization to make the change you are requesting.  Our fax number is on the form or if you wish to mail in your form, please mail it to:

Once your EFT bank account change has been processed, you will receive a confirmation letter indicating the effective date of the change.  Please, do not close your old EFT bank account or start using your new EFT bank account until you receive the confirmation letter.  If you have any questions, you may contact us by calling the phone number on the attached form.
