Diabetes Primary Prevention Initiative
Michigan was one of five states to receive CDC funding to pilot diabetes primary prevention activities as part of the Diabetes Primary Prevention Initiative (DPPI). Michigan developed partnerships within the DPPI Intervention focus area, namely with WISEWOMAN, WIC and the Northern Michigan Initiative. WISEWOMAN Diabetes Primary Prevention Project
Michigan's Lenawee County Health Department is completing a pilot project to integrate prediabetes screening and to increase links to community-based lifestyle programs for nutrition and physical fitness into its WISEWOMAN program. Prediabetes screening in the WISEWOMAN program expanded statewide in 2008.
Northern Michigan Diabetes Initiative (NMDI)
Munson Healthcare, Priority Health and TIPDON (Northern Michigan Diabetes Outreach Network) are implementing a 10-year diabetes initiative to improve the care of people with diabetes and to increase awareness and testing for prediabetes in 11 northern counties. A team/collaborative community approach is used to increase health care provider capacity to prevent, detect, and treat diabetes and prediabetes, and to increase public awareness of diabetes prevention.
West Michigan Diabetes and Cardiovascular Health Partnership
The Michigan Diabetes Prevention and Control Program works with diabetes and cardiovascular agencies in the Grand Rapids area to integrate messages and community efforts. Partners include: American Heart Association, American Diabetes Association, Spectrum Health, National Kidney Foundation of Michigan, TENDON (regional Diabetes Outreach Network), Kent County Health Department, St. Mary's Health, Pfizer, and the Michigan Department of Community Health Diabetes and Cardiovascular Disease Units.
