The PATH (Projects for Assistance in Transition from Homelessness) Grant is funded by the McKinney Homeless Assistance Act and provides funds for services to persons with mental illness who are homeless or at imminent risk of homelessness. There are currently 19 projects operating in the following county community mental health agencies:

• Bay-Arenac CMH,
• Berrien CMH,
• Summit Pointe (Calhoun County),
• Clinton-Eaton-Ingham CMH,
• Copper Country CMH,
• Detroit Wayne CMH (three subcontract agencies),
• Genesee County CMH,
• Lifeways, Inc. (Jackson County),
• Kalamazoo County Human Services Department,
• Kent County CMH,
• Macomb County CMH,
• Monroe Community Mental Health Authority,
• Muskegon County CMH,
• Oakland County CMH (two subcontract agencies),
• Saginaw County CMH, and
• Washtenaw County CMH.

These projects combine aggressive outreach and innovative interventions in ways that are designed to meet the needs of individuals who are homeless and mentally ill. In general, clients get training and assistance within assessing supports for continued community living. These services include, but are not limited to:

• help in assessing income supports such as federal disability income or veteran benefits,
• assistance in locating affordable housing which may include financial assistance with damage/security deposit, first months rent and utility deposit assistance when needed,
• help in moving and assistance in obtaining household furnishings and supplies to help set them up, and
• linkage to appropriate medical, mental health and educational services.
