ODCP Survey - Help improve our website

How can we serve you better? The Michigan Department of Community Health/Office of Drug Control Policy (ODCP) is dedicated to providing the best possible service to all who visit our site. We would like your opinion and feedback on your visit to our website so that we might redesign it in a manner that is more user friendly and is more efficient for you and for others. We hope you take the time to help us by completing the following survey.

The first four questions are intended to tell us who is visiting our website and what are our visitors are looking for. There is also a comment section at the end of the survey that gives us an opportunity to hear from you on what we might do to improve our website.
