  • When do I need to apply for the Family Medical Leave Act (FMLA)?

    Apply for FMLA and provide medical documentation to DMO if:

    • You will be off work for five (5) or more days for a serious health condition for yourself, spouse, parent or child.
    • You will be off work intermittently or on a reduced schedule due to a serious health condition for yourself, spouse, parent or child.
